Upload a copy of your GoPass identification badge showing your employer, name, photo and GoPass sticker (.pdf, .jpg, .png, .gif). File cannot exceed 10MB.

If your ID badge is two-sided, please be sure your file contains both sides in the single file being uploaded. We can only accept one upload.
